exosmart™ Academy

Everything We Know About exosmart™— and Everything We Do Not.

exosmart™ concentrates a patient's own plasma at the point of care, without separating it. The vesicles, the growth factors and the plasma proteins that carry them stay together and are concentrated as a single fraction.

This Academy covers the science behind that, the evidence for it, and the point where the evidence stops. We make the device, so every claim names its source and every section states its own limits.

Doctors and distribution partners are taught separately. Both tracks are free; clinical modules open once we have verified your medical registration.

Free access. Evidence-based. Certificate of completion.

WHAT WE KNOW: The Whole Signalling Fraction Stays Together

Blood does not deliver a single molecule to injured tissue. It delivers a mixture — vesicles, growth factors, signalling proteins and the plasma proteins that carry and protect them — and they arrive together because that is how the body sends them.

Almost every preparation in this field takes that mixture apart. A platelet concentrate keeps the platelets and discards the plasma around them. A vesicle isolation keeps the vesicles and washes the proteins away, because the membranes used for that job pass everything below several hundred kilodaltons. Each of those choices throws away a fraction that was there for a reason.

exosmart™ makes the opposite choice. The membrane holds back everything above roughly 15 kilodaltons, so the vesicles, the platelets, the growth factors, the signalling molecules and the larger plasma proteins all stay in and are concentrated together.

exosmart™ is used by a doctor as part of a procedure. Blood is taken, the plasma is prepared, and a trained doctor gives the concentrate to the patient. Any procedure involving a needle in a vein and a needle in tissue carries risk, and the preparation is not suitable for every patient. The instructions for use set out which patients should not be treated, what to watch for, how to select patients and how to handle the device, and the doctors' training covers all of it.

In a prospective study of 30 patients in Alessandria there were no serious adverse events.

Nothing is separated out, nothing is purified, and nothing is selected. The patient's own plasma goes in whole and comes back concentrated and whole. This is the complete secretome, and keeping it intact is the design rather than a limitation of it.

The preparation is autologous throughout, and no chemical activator is added.

WHAT WE DO NOT KNOW

How much of each individual component ends up in the final preparation. Concentration is a volume relationship, and some protein is always lost to the membrane surface and to the volume held inside the device. We have not measured recovery component by component, so we do not put a single multiplication figure on it.

WHAT WE KNOW: The wider research, and why it is not a verdict on this device

Autologous platelet preparations have been studied for thirty years, in thousands of patients, across orthopaedics, dermatology, hair and gynaecology. Some trials found meaningful benefit. Others, well designed and properly controlled, found none. Both belong in the same module, and in this Academy they get the same weight and the same size type.

Professional bodies disagree with each other about the same evidence. What is worth noticing is the reason those recommending against give: not that these preparations were shown not to work, but that the studies are of variable quality and the preparations themselves have never been standardised. That reasoning cuts in both directions. If the class is too varied for its positive trials to settle the question, it is equally too varied for a negative class finding to settle it for a preparation made a different way — and by the same logic we cannot claim the positive trials as ours either.

WHAT WE DO NOT KNOW

Whether results from any of that literature transfer to this device. They were produced with different preparations, different concentrations and different protocols. We teach them as what they are — evidence about platelet-rich plasma — and never as evidence about exosmart™.

Five Rules of Evidence

We publish what we have measured in exosmart™, what has been observed in related preparations, and what wider research can and cannot tell us. These five rules keep the evidence clear and the conclusions proportionate.

1

Name the
evidence level.

Every claim is identified as device-specific, device-family, platform-level or adjacent. The categories are relevant, but not interchangeable.

2

Report what was actually measured.

Laboratory measurements describe the preparation. They are not converted into clinical outcomes unless a clinical study measured that outcome.

3

Keep variability visible.

Ranges stay ranges. A high-performing sample is not presented as typical product composition. Context stays with every number.

4

Trace every figure
to its source.

Studies, third-party laboratory reports and controlled documents are identifiable behind the claims.

5

State the limit
where it matters.

State what the evidence establishes, then state the relevant boundary once. Do not let the limitation obscure the supported finding.

WHAT WE KNOW: About Risk

WHAT WE DO NOT KNOW

Thirty patients is a safety observation, not a safety database. There is no long-term follow-up and no head-to-head safety comparison against another preparation. Diagnosis, patient selection and the decision to treat stay with the treating doctor, and nothing in this Academy replaces that judgement.

What you will learn, depending on why you are here

A doctor has to decide which patient to treat and, more often, which patient not to. A partner has to carry the science into a conversation without overstating it, because the doctor will check. Both tracks start from the same evidence and separate after it.

The science first, then the regulatory position, where the device sits among other preparations, and how to get a doctor through a first case. The clinical modules are open to read, so you can answer a question in the room instead of promising to come back. Treatment detail stays in the doctors' track.

What the certificate proves, and what it does not

Modules with a test lead to a certificate of completion. It records that a named person passed a named test on a date, and you can download and keep it, which is what makes it useful as a training record. The name is typed in by the learner, so it is not a secure or independently checked credential.

No outside body accredits it. It is not a medical licence, not a specialist qualification, not clinical privileges, not CME or CPD credit, and not a regulatory approval of any kind. If a hospital, a distributor or an authority asks what it proves, that is the honest answer, and it is better coming from us than discovered later.

This Academy grows with the evidence.

New modules are added as new data is verified —never before.

The device. exosmart™ is a CE-marked Class IIa medical device under EU MDR 2017/745, Rule 3. It is a single-use cartridge that concentrates plasma by filtering it. Legal manufacturer: Medica S.p.A., Via Degli Artigiani, 7, 41036 - Medolla (MO) - Italy. CE certificate MDR 00048-A. Notified body: Kiwa Cermet Italia (NB 0476). Valid until 31 May 2028. What the CE mark means. It shows the device meets the EU MDR rules for the purpose written in its documentation. It is not permission to treat a particular disease, and it does not prove the device works for any disease. What the device is registered for is different in each country, and it is not sold in every country. Check the instructions for use and the registration in your own market.

Risk. exosmart™ is used as part of a medical procedure and carries risk. The instructions for use list the warnings, the precautions and the patients who should not be treated.
